Empath Health is a nonprofit integrated network of care that provides expert, life-changing health care to those with chronic and advanced illnesses. Empath Health includes hospice, palliative home health and physician services, independent living assistance and day care for seniors, HIV services and advance care planning.



Empath Home Health is a nonprofit provider of comprehensive home health care services based in Pinellas County. As a member of Empath Health, our focus is helping homebound patients and their caregivers manage their illness or condition and enjoy a better quality of life.



We are currently seeking an experienced and compassionate Home Health RN to be responsible for identifying and coordinating patient/family needs and for providing palliative, preventative and rehabilitative care to the patient/family unit. The RN utilizes assessment, intervention and teaching skills to mazimize patient comfort and to enhance the quality of life to the patient/family unit. The RN follows all home care regulatory and compliance requirements and meets all Joint Commitment standards.

Position Requirements




--- Education and/or Experience: Associate's degree (A.A.) or equivalent from two-year college or technical school; three (3) years nursing experience, with at least one (1) year acute care setting preferred; or equivalent combination of education and experience.

• 
  
• Minimum of one (1) year home health experience with thorough understanding of State, Federal and Joint Commission Home Health Agency Regulatory Standards preferred.
  
• Licensure:
  
  o Valid Florida license as a Registered Nurse
  
  o Valid Florida State Driver's License
  
  
  • 
    
  • Continuing Education: As required for licensure.
    
  • Certifications: CPR required
    
  • Post Offer Employment Testing (POET) or Fit for Duty examination (as necessary) with "Capable" result required.
    
  • Intravenous therapy training and experience preferred.
    
  • Preferred experience caring for terminal patients and others with life threatening illnesses.
    
  • The ability to type 25 wpm is highly preferred. Must receive a passing grade on a pre-employment computer literacy evaluation.
    
  • For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee's HR file.
