Seeking a compassionate Physical Therapist to provide high-quality care to patients in their homes, with a focus on improving mobility, reducing pain, and restoring function.

About the Job
As a Physical Therapist with Pinnacle Home Care, you will be responsible for assessing patients' conditions, developing personalized treatment plans, and delivering therapy to help improve their quality of life. You will work closely with patients, families, and healthcare teams to ensure seamless care and achieve optimal outcomes.

Responsibilities

• 
  
• Conduct comprehensive assessments of patients' needs, including Outcome and Assessment Information Set (OASIS) assessments.
  
• Develop and implement individualized treatment plans in consultation with physicians and revise as necessary.
  
• Collaborate with interdisciplinary teams to develop and implement client care plans.
  
• Treat patients to relieve pain, develop or restore functional abilities, and maintain maximum performance using physical means.
  
• Teach, supervise, and counsel patients and families on physical therapy programs and adaptive devices.
  
• Record and report evaluation data, treatments, and patient responses to therapy interventions.
  
• Participate in case conferences, staff meetings, and in-service programs as necessary.
  
  
  Requirements
  
  • 
    
  • Graduate of a Physical Therapy school approved by the Council of Medical Education of the A.M.A. in collaboration with the American Physical Therapy Association.
    
  • Licensed or registered by the State.
    
  • At least one year of experience in an acute/sub-acute setting.
    
  • Home care experience and supervisory experience preferred.
