Job Description:
AccentCare is seeking a dedicated and compassionate Social
Worker with MSW and LCSW credentials to join our Hospice team. In
this role, you will provide crucial support to individuals and
their families during end-of-life care. Your expertise in
psychosocial support and care planning will aid in enhancing the
quality of patient life, aligning with AccentCare's mission to
deliver personalized and compassionate care. Your responsibilities
will include conducting assessments, providing counseling, and
creating tailored care plans to meet the unique needs of each
patient. Assess patient psychological and social needs to develop
personalized care plans. Provide counseling and support to patients
and their families to help them navigate end-of-life challenges.
Collaborate with interdisciplinary teams to ensure a comprehensive
approach to care. Educate families and patients on available
resources and options. Document patient care and progress according
to regulatory and organizational standards. Advocate for patient
needs to ensure optimal care. Strong interpersonal and
communication skills Proficiency in behavioral therapies Advanced
knowledge in end-of-life care processes Ability to handle sensitive
situations with empathy Experience in crisis intervention Medical
Insurance Dental Insurance Vision Insurance 401k Licensed Clinical
Social Worker (LCSW) required. Certification in Hospice and
Palliative Care (optional but preferred)
